Claim Procedures

Either your healthcare provider will bill your insurance directly, or you will be required to fill out a claim form . You will then receive an Explanation of Benefits detailing how your claim was paid. This is not a bill. At the bottom of most EOB's is an amount labeled "patient responsibility" - this is the amount owed to the healthcare provider. It is then your responsibility to pay this amount to the provider directly.
