ID Card Anatomy

The following is an example of an SRC ID Card. Your individual card may be somewhat different, according to coverages offered. Please contact you claims administrator if you have any trouble understanding your ID Card.

Please note that an ID Card is not a guarantee of coverage. Coverage is contingent on the payment of your premiums.

Prescription Drug Benefits

A. Prescription coverage is provided by a national vendor who is able to make sure that you get coverage in pharmacies across the country.

B. Contact Information. You can contact your prescription vendor to ask them about your coverage or to locate a pharmacy.

Accident and Sickness Benefits

C. Coverage Dates. Coverage is available for the dates specified only.

D. Member ID number. This number is used to uniquely identify you as a member of your health plan. You may be asked for this number when contacting customer service.

E. Policy Number. This number identifies your school plan's policy number, and can be used throughout this site to access information specific to your coverage.

F. Your information. It is important that this information is correct. If your address is incorrect, please use our online address change utility.

G. Provider Network. Your plan may have a network of preferred physicians. You may find that your benefits are considerably better for participating providers. Read more about Provider Networks.

H. Claims address. If your provider does not file the claim themselves, you will need to mail a claim form to this address.
